Supporting Seniors With Care, Connection, and Stability
CHJCC’s Senior Support Services are dedicated to helping older adults remain healthy, secure, and connected as they age. Navigating benefits, healthcare systems, and daily needs can be overwhelming. Our goal is to simplify that process and ensure seniors have access to the resources and support they need to maintain independence and quality of life.
We work closely with seniors and their families to provide guidance, referrals, and ongoing assistance, always with warmth and respect.
Services Include:
How to Apply for Services:
Assistance with Medicare, Medicaid, and prescription coverage programs
Support applying for rent and property tax relief programs
Referrals for in-home care, legal assistance, and social services
Help accessing SNAP, SSI, and other public benefits
Case management and resource coordination
Seniors aged 60 and older, or their caregivers, are encouraged to reach out for support. Our staff will take the time to understand each individual’s needs and connect them with appropriate services. Appointments can be scheduled by phone or online, and assistance is provided in a patient, supportive manner.
