ENTITLEMENTS & SOCIAL SERVICES OVERVIEW

The objective of the entitlements and social services department of the CHJCC is to help families and individuals with social and economic needs lead independent lives and enhance the quality of their life with possible help from various government or private agencies.

This statement is pretty dry and impersonal; however in our daily encounters at the office this need becomes very human, real and very much necessary.

"My elderly parents live alone in Brooklyn, they manage now, but I realize they will need more and more help very soon, what do I do and where do I start"…

'My wife is having baby number …we can not afford much help, the family is so stressed out, if we would have some cleaning help it will make a tremendous difference.

"I fell and broke my arm, I can not work for at least the next few months...my rent is due."

"I am in my 3rd/4th/ 5th month, etc., and I just now realized that I have medical insurance coverage for my self but not yet for my unborn baby…………."

"I lost my Medicaid card; the doctor won't see me……."

"I got this bill…..I have no clue what they want from me……"

These and many many others are the usual requests we receive all the time.

We will pre-screen you and your family for most entitlement programs, such as: Medical Insurance, Food Programs, Public Assistance, Senior Services, SSI & Social Security benefits, HEAP and related programs, emergency assistance, tax benefits and more.

    * Help you to navigate the maze of government offices when applying for services you might need and qualify for. Get an appointment for you at the proper office.
    * Prepare the appropriate application for the programs you are applying for.
    * Help you to prepare the right documentation as needed.
    * Follow up on your application and intervene on your behalf.
    * Help with correcting benefit related errors.
    * Advocate for you and the services you have applied for
    (applying for fair hearings and representing senior and disabled clients when needed).
    * We provide case assistance to the Senior population with variety of senior programs and services, Home care programs, SCRIE (senior citizen rent increase exemption),
    Medical Insurance programs (Medicaid, Medicare, Medicare Buy In, EPIC, HIICAP (health insurance counseling for seniors), Transportation Programs (access-a ride, MTA reduced fare metro card) emergency Financial assistance to seniors to help pay for housing or other emergency needs, when there is not other resources of help available.

Our Mothers Helper Program provides house keeping help for new mothers in our community. Many women in the community benefit from this unique Program, where a housekeeper comes in weekly for about 6 weeks to help the mother of the family recover after giving birth or when such help is needed for health or other family reasons.

Emergency financial assistance to families and individuals when entitlement resources are exhausted or not available.

We would like to stress that all information discussed with our social service workers are kept strictly confidential.
